TRICHINOSIS: ACUTE


Cyst with larvae: Active phase of infection


Congo red stain

Gomori trichrome stain
  • Larvae are in cysts within muscle
  • Each cyst contains one coiled larva
  • Cysts also contain "nurse" cells with large nuclei (Arrow)
  • The outer edge of the cyst is a well-defined wall (Arrowhead)
  • There is little inflammation around well-formed cysts


Alkaline phosphatase stain

PAS stain
  • Contents of cyst around larvae stain with alkaline phosphatase.
  • Cyst wall and larva contain carbohydrate and stain with PAS.


Inflammation near unencysted larvae


H&E stain

Acid phosphatase stain
  • Inflammation near larva may include multinucleated giant cells
  • Many of the cells in the infiltrates are histiocytes (macrophages) that
    • Contain nuclei that are larger and paler than those in T-cells (Left)
    • Stain with acid phosphatase (Right)
